Liability waiver

I understand that participation in martial arts training involves inherent risks of physical injury. I voluntarily assume all such risks and release Raymond Martial Arts Academy, its instructors, and staff from any liability for injuries or damages sustained during training, classes, or events.

I confirm that the participant is in good physical health and able to participate. If registering a minor, I confirm I am the parent or legal guardian and consent to their participation.
